Construction

Rocky Mountain Rail Park WWTF

Issue: 

  • Pipe sleeves were not installed on pipe penetration through concrete slab

Resolution:

  • IWS accepted liability for any pipe failures due to missing pipe sleeves

Lesson Learned:

  • All pipe penetrating concrete shall be installed with pipe sleeves, the sleeve shall be two pipe sizes greater than the pipe passing through the concrete and the annulus between the main pipe and the sleeve shall be filled with non-adhesive flexible material to allow the pipe to expand and contract. 

Design

Rocky Mountain Rail Park WWTF

Issue: 

  • S-traps were designed for plant drains

Resolution:

  • Field services modified drain piping to accommodate P-traps

Lesson Learned:

  • P-traps should be used in design not S-traps

Tejon WWTF

Issue: 

  • Wrong type of DI pipe specified for potable water lines

Resolution:

  • Ordered the correct pipe during construction

Lesson Learned:

  • DI pipe with 401 lining is for sanitary sewer and waste water applications only, if using DI pipe for drinking water must be lined with cement-mortar or porcelain enamel (glass)

Tejon WWTF

Issue: 

  • Flanged tee used for A/V valve when saddle tee would have been better

Resolution:

  • No resolution applied in this case

Lesson Learned:

  • Use saddle tees for A/V valve on large pipe

Manufacturing

All WWTF With Platforms

Issue: 

  • Carbon steel bolts used and shipped with structure

Resolution:

  • Ordered the correct pipe during construction

Lesson Learned:

  • All wastewater steel structures should be assembled with 316 stainless steel bolts

Rocky Mountain Rail Park WWTF / Tejon WWTF

Issue: 

  • Body of headworks screen leaked during testing (Screen provided by Saveco)

Resolution:

  • Saveco mobilized crew to repair screen body

Lesson Learned:

  • Require suppliers to provide QA documents that all equipment has been checked prior to shipping

Procurement

Tejon WWTF

Issue: 

  • 8 1/2 inch long flanged pipe spool was ordered, supplier shipped 8 1/2 foot spool was manufactured and shipped

Resolution:

  • Spool had to be re-ordered delaying installation

Lesson Learned:

  • Make sure quotes and submittals from suppliers match the order

Tejon WWTF

Issue: 

  • Hydropneumatic tank delivery delayed, causing delays in commissioning water treatment plant

Resolution:

  • Project delayed, owner had to extend operation of temp plant

Lesson Learned:

  • Identify long lead items at beginning of project and check out new vendors with references.

Project Management

Rocky Mountain Rail Park WWTF

Issue: 

  • Excavation contractor told to perform extra work that was not in his original bid with out an official change order.

Resolution:

  • Negotiate payment after job complete

Lesson Learned:

  • Ensure all extra work is approved by official CO prior to authorizing the extra work

Safety

Multiple

Issue: 

  • Safety ladders with tie-off cables are cumbersome to use and present a hazard

Resolution:

  • No resolution applied in this case

Lesson Learned:

  • Use safety cage instead
